Foster Home Commitment

Please read the following carefully as you will be asked to consent to all items listed on this document. Please print or copy and paste into a document to save for your references.

We appreciate our foster homes, and this arrangement gives a person in crisis the comfort of being able to seek the support they need and deserve while feeling confident their animal is out of harm’s way and in a loving home, until the time comes to be reunited. The policies below are created to ensure the safety and health of both the foster animal and your resident pets. These policies must be agreed to and met in order to foster for the Cleveland Animal Protective League’s Crisis Safety Net Program (going forward will be referred to as CSN).

  1. I agree that, when taking a foster animal, I will maintain possession of the animal(s) for the duration of the agreed upon time (30-90 days). If I am unable to continue fostering, I will arrange with the Project CARE Team to bring the animal back to the shelter to be sent to a new foster. I will not give my foster to another party under any circumstances.
  2. If the Crisis Safety Net foster must be kept separate from my pets, I agree to permit a representative of the Cleveland APL to visit my home and observe where the foster animal(s) will be kept.
  3. I assume all responsibility for the health and safety of my pets and will not hold the Cleveland APL accountable for any medical or other expenses if I choose to allow my foster pets to interact with my own pets.
  4. Upon request by the Cleveland APL, I will return the foster animal(s) to the shelter within 24 hours or less.
  5. I agree to foster the animal(s) until the pet can be reunited with its owner (maximum of 90 days).
  6. I agree to schedule with Project CARE when I will be bringing the foster animal(s) in for any reason (revaccinations, checkups, blood tests, or return to their owner). If I cannot make my scheduled time, I agree to notify the Project CARE Team as soon as possible in order to reschedule for the next most convenient time.
  7. If a Crisis Safety Net foster animal gets sick, I will notify the Project CARE team and bring it back to the shelter for a medical examination as soon as possible.
  8. I agree to provide any necessary medications to my Crisis Safety Net foster animal(s).
  9. I assume all financial responsibility for the Crisis Safety Net foster animal if I take them to any veterinarian other than the Cleveland APL when it has not been approved by the proper Cleveland APL staff, including unauthorized emergency vet visits.
  10. I agree that my Crisis Safety Net foster animal(s) are owned by another party and that I will bring them in on schedule and agree to any medical decisions the APL veterinarian finds the most valuable for my foster animal.

Privacy Agreement: Because of the nature of the situation, confidential information may not be shared in any form including but not limited to; verbal, written, computerized data, etc. Confidential information includes but is not limited to; case circumstances, names or addresses of people and animals involved, pictures, etc. In all cases, fosters are prohibited from disclosing or discussing such information outside of the proper personnel at the APL or representing themselves as a spokesperson for the APL unless specifically authorized to do so by the CEO.

By typing my name below, I am stating I have read and agree to all of the standards listed in the Crisis Safety Net foster home commitment. I understand that, should I consistently fail to follow these standards, I will be removed from the Crisis Foster Net foster program. I am also giving the Cleveland APL permission to verify any of my requested information.
