Amphibian Survey

Please only submit a survey if you are interested in a specific animal you see listed on our website. Unfortunately, we cannot take waiting lists for animals that are not currently listed as available.

Once you submit your adoption survey a team member will reach out to you via the telephone number provided on your survey to discuss the animal you are interested in and the next steps in the adoption process. We will respond to inquiries based on the timestamp of the survey receipt, and it may take up to 24-48 hrs to receive a call.

  • By submitting this application you agree to the following policies:

    1. The adopted animal will live as a companion and will not be used for breeding.

    2. The animal will not be released into the wild, nor will it be taken outside for any reason other than safe transport in an enclosure to a vet.

    3. The animal will receive routine and emergent care as needed from a qualified veterinarian at the owner’s expense.

    4. The animal will be kept in humane conditions appropriate to its age, health and behavioral needs.

    5. The Cleveland APL reserves the right to refuse an adopter’s application for any reason at any time during the adoption process.

    I, the applicant, agree to adhere to the above adoption policies and affirm that information provided is true to the best of my knowledge. I understand that if approved for adoption I am expected to make substantial commitment of my time and money for the length of the animal’s life, which may exceed ten years.
